Primary Care (Doctors & Nurses)

Rexdale CHC offers a family practice medical services centre in which family doctors and nurse practitioners work together in teams. An emphasis is placed on health promotion and prevention. Primary Health Care services at Rexdale CHC provides the first point of contact of health care and family medicine by a multidisciplinary team of primary care providers including family physicians, nurse practitioners and registered nurses. Services include disease cure, management and prevention, rehabilitation and health promotion.

• One-on-one appointment for individual client
• Groups with specific themes and objectives for clients and community members (pre-registration is usually required)
• Special health education activities for clients and community members (pre-registration maybe required)
• Pregnancy care till 32 weeks
• Immunization for babies & young children
