Senior Case Manager (AMANI Mental Health & Substance Use Program)

The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds.
Rexdale Community Health Centre is expanding the services available to African Canadian & Caribbean Youth through its partnership with the Centre for Addictions and Mental Health- CAMH and its AMANI Mental Health & Substance Use Program.
AMANI provides culturally responsive services and care to youth (ages 12-29) from Black communities. AMANI’s model is rooted in holistic, healing-centred, Afrocentric, and culturally responsive care. Offering a range of prevention, early intervention, harm reduction, and recovery-oriented services to assist youth and their families/caregivers in reducing harm, moving toward recovery, and making healthy choices for themselves and their families. The program places focus on the full spectrum of social determinants of health and is grounded in shared community values. The service also recognizes the resiliency and strength, cultural diversity and
intersectional experiences of Black youth. AMANI is individually tailored to respond to the lived experiences of anti-Black racism and systemic oppression.
This position will work largely with Black youth/clients and their families, as appropriate, who are dealing with mental illness and/or co-morbid addiction and, as necessary, provide case management support and referrals to other providers serving this population. The successful incumbent will deliver care within the AMANI model that recognizes the lived experience of Black youth who have faced anti-black racism and oppression as it relates to the many aspects of their lives, including the social determinants of health – housing, unemployment, and education. Areas of work will include intake, case management, outreach, transition support, systems navigation and group therapy.
Responsibilities:

• Respond in a timely manner to information/referrals of Black youth and their caregivers who are impacted by mental health and/or co-morbid addiction experiences
• Conduct a comprehensive assessment of the client’s situation and/or response to referrals received
• Incorporate an anti-racism and anti-oppression lens in all assessments and response, including the impact of social determinants of health
• Ensure assessment and service plan incorporates clients’ goals, values, skills, resources, strengths and service requirements from a self-determination principle
• Ensure clients’ linguistic and cultural needs are considered in the service plans
• Provide Black youth and their caregivers with general information, contacts, referrals services and support as needed to facilitate access to various health, mental wellness and other social services opportunities that promote the social determinants of health, including, but not limited to: housing, employment, education, health, including substance abuse, social abuse, social and emotional issues, etc.
• Provide crisis counselling, short-term crisis management, advocacy and referrals involving the client and members of his/her immediate support network in the process
• Work collaboratively with clients to identify, develop and fulfill the goals they identify as meaningful and relevant
• Foster and assist in the development of self-reliance and strong peer connections
• Liaise with service-providing agencies, including mental health, substance use, and justice initiatives, in order to maximize support to the client
• Follow up with clients as well as service providers to ensure uptake of services and that the relationship is working, including organizing case conferences of involved clients, supports
• Inform clients about resources to facilitate self-referrals
• Assess clients’ familial and social networks and identify/offer additional support to caregivers and support networks
• Maintain an up-to-date knowledge of community resources, internal policies and procedures, agency protocols and share relevant information with other staff
• Participate in staff meetings and on interdisciplinary teams; participate in the Centre as may be required
• Develop, implement and update the case management work plan in conjunction with the supervisor, as well as identify action plans
• Keep general program and participant information and records, and maintain accurate engagement and outreach statistics, records and reports; ensure weekly data collection and recording of all encounters, either electronically or manually, as directed
• Develop and maintain a flexible work schedule that accommodates the needs of the community and the Community Crisis Response Service, as approved by your supervisor
• Carry out other duties as may be assigned from time to time

Qualifications and Assets:
• A post-graduate degree in Social Work, Child and Youth Care, other Social Service, or a combination of at least three years of related work experience and relevant education.
• Three to five years’ work experience in social service positions working in a culturally and racially diverse community.
• Demonstrated skills and experience in working with youth of African descent
• Demonstrated commitment to and lived experience of anti-Black racism and Afrocentric values and principles;
• Knowledge of community resources for African Canadian and Caribbean youth and their families in substance abuse treatment is essential.
• A deep, fundamental, and comprehensive knowledge of the unique challenges faced by African Canadian and Caribbean youth and their families across the social determinants of health.
• Ability to work with Black youth to develop and enhance their identity, culture and sense of belongingness to their community.
• A commitment to an interdisciplinary team approach to the coordination of health and social services
• Strong communication skills (verbal and written) combined with computer literacy are required. Ability to negotiate, communicate and maintain a supportive, trusting relationship with those being served.
• Experience in program development, implementation, and evaluation.
• Knowledge of community-based health services
• Strong and demonstrated ability to form relationships with individuals while maintaining healthy boundaries
• Caseload management skills, including developing action plans for clients, monitoring client progress and maintaining records
• Ability to work effectively within a diverse and multi-disciplinary team.
• Excellent interpersonal, decision-making, problem solving and conflict management skills.
• Demonstrated flexibility, organizational and time management skills in a high-pressure environment.
• Ability to work evenings and occasional weekends.
• Knowledge of Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
• Knowledge of the Rexdale community
• Bilingual in French and English considered a strong asset
• Must be able to provide a vulnerable sector security clearance

Salary Range: $ 65,327 to $ 78,704 annually (based on experience), plus HOOPP and benefits
Hours per week: 35
Status: Unionized
