Primary Health
Care @ Rexdale C.H.C.
A
family practice medical services centre in which family doctors
and nurse practitioners work together in teams. An emphasis is
placed on health promotion and prevention.
Rexdale C.H.C. uses a multi-disciplinary
approach to health and well being. A range of services are offered
through an integrated approach to wellness which links clients
to the programs they need to maintain good health.

Health Clinic:
Rexdale
CHC operates two Sexual Health Clinics in the community. Both
are in partnership with Toronto Public Health. Sexual Health Clinics
exist so that every one can access information, education,
treatment and medications that will help them live sexually healthy
lives.
Services available include:
- Birth Control
- Sexually Transmitted Infections
- Pregnancy Testing and Counseling
- Sexuality Questions
- Hepatitis B Vaccine
- HIV Testing and Counseling
Sexual Health Clinics take place in two
different locations:

Healthy Eating Groups Presentation:
Rexdale Community Health Centre offers healthy group
presentations that will encourage on-going physical activity
and nutritional behaviour changes designed to support life-long
fitness and good eating habits. Rexdale Community Health
Centre believes that the implementation of healthy eating for
community groups that incorporate nutrition education and improve
the quality of foods served are the best way to educate the Rexdale
community to pursue healthy eating habits and promote a healthy
diet.
Topics generally cover Canada’s Food Guide
to Healthy Eating;
1. Food Budgeting & Grocery shopping,
2. Menu planning,
3. Cooking skills
4. Can be adapted to the diversity / multi-cultural needs of
your community.

Chiropody:
Chiropody Clinic (Foot
Care). The practice of Chiropody is the
assessment of the foot and the treatment and prevention of diseases
or disorders of the foot by therapeutic, orthotic and palliative
means. This service specializes in providing complete
family foot care for diabetic children, seniors and adults.
The services include: provision of orthotics & orthopaedic
shoes, physical therapies, sport medicine, nail care and foot
care.
A Chiropodist trained in all aspects of care for
the feet and lower limbs, including the most common foot problems
below:
- Geriatric care
- Sports injuries
- Common foot conditions such as corns, warts, ingrown toenails,
or calluses
- Foot infections caused by medical conditions such as diabetes
- Foot deformities and misalignments both in adults and children

- Biomechanics exam of the foot and leg,
- Physical therapies,
- Foot surgery consultations,
- Wound care,
- Nail surgeries

- Orthopedics Shoes

Diabetes Education Centre:
The Community based Diabetes Education
Centre
A team of professionals experienced in diabetes care, consisting of Dietitians and Nurses, offering FREE education to clients on how to manage and/or prevent Type 2 diabetes. We provide individual counselling and group education sessions for clients with Type 2 diabetes and pre-diabetes at flexible hours, including evening and weekends. No physician referral or health benefits are required, self referral is encouraged. Refugees and new immigrants are welcome. Nutrition Counseling:
Community
Dietician provides individual counselling on a range of nutritional
issues: prenatal, infant feeding, weight management,
diabetes, high cholesterol, and more. Offers group education to
parents, staff and other community groups.
Examples of Community Groups:
- Multi-cultural wellness cooking classes
- Healthy heart program
- Low fat cooking class

Good Food Box
The Good Food Box is a program designed for people
who want to include fresh fruit and vegetables in their diet.
The Good Food Box distribution system provides low-income people
with improved access to nutritious foods in their community, while
also encouraging community development, and support for the local
agricultural economy.
Twice a month on Tuesdays
Cost $17-$32

Home Visits:
Family Physicians provide home visits for
palliative care clients. By utilizing home visits, physicians
improve the health and quality of life for the client. Physicians
will schedule regular home visits to insured that the patient
is receiving consistent care. The patient will no longer have
to worry about transportation in order to receive medical care.
By emphasizing education and prevention, emergency
room visits and admissions can be significantly reduced. Furthermore,
when appropriate, physicians will utilize other healthcare providers
including home nursing and home pharmacy.

7.
Annual Flu Clinics:
Every year, on average, 25% of Canadians get the
flu. Protect yourself and those around you by getting the flu
shot.
Rexdale Community Health Centre provides two
free Flu Clinics every year during the month of November
in partnership with Toronto Public health.

Well Woman Clinics:
Emphasis on women's
health care is one of the priorities at Rexdale
Community Health Center. Female clients can book well women visits
with a female health provider. Routine well women care includes:
- discussing your medical history
- any medical conditions you have
- medications
- breast health
- self-breast examinations
- annual pap smears
- family planning
- diet
- exercise
- alcohol and/or drug use
- depression or anxiety
- any other problem you may be having
Well woman clinics are coordinated by Nurse practitioners.

9.
Pre-and Post Natal primary care for women and newborns:
Rexdale Community Health Centre offers prenatal,
post natal care and obstetrical services for registered
clients of the centre. Prenatal care uses a share care model with
obstetricians. Prenatal care is offered by nurse practitioners
and physicians, and clients are referred for delivery to an obstetrician.
Perinatal Education: provides
health education in the perinatal period through a variety of
educational strategies and approaches. Core education targets
healthy pregnancy, childbirth preparation and breastfeeding, and
early parenting.

Monday 12-8 pm
Tuesday – Friday 9-5 pm
One Saturday a month 9-2 pm
