Diabetes Education Centre

Rexdale CHC’s Diabetes Education program offers FREE services to individuals, groups, caregivers and employers to assist people living with diabetes to better manage the disease and prevent complications that can limit ones independence. We also counsel people with pre-diabetes to delay the onset of diabetes.

Programs and services include:

  • Diabetes support group
  • One-on-one counselling sessions with a Registered Nurse and Registered Dietitian
  • Diabetes drop-in
  • Meter trainings
  • Insulin starts
  • Community screenings
  • Exercise programs
  • Nutrition and Heart Health presentations
  • Foot care
  • Grocery store tours

Individuals and area we serve: People diagnosed with Type 2 diabetes and pre-diabetes living in the north west corner of the GTA. Refugees and newcomers are welcome. Doctor’s referrals and self-referrals accepted. Health insurance not required.
