Operating out of three locations, RCHC offers core services that include primary health care, health promotion, community development programs, Pathways to Education, and a Diabetes Education Centre.<\/p>\n<p>The TO Wards Peace Project (TWP) is a risk-driven initiative made for Toronto, focused on addressing the elevated level of gun violence in Northwest Toronto. The TWP Project Lead will oversee and support the implementation of the project in the Rexdale area. This will include contributing to program design and development, supporting interventions, conducting research and documentation, facilitating representation and partnership development, and providing training, mentoring, and organizational learning for implementing Youth Peace Projects.<\/p>\n<p>We are looking for an individual with project\/program development, implementation and supervision experience to support this initiative.<\/p>\n<p><strong>Responsibilities:<\/strong><\/p>\n<h3><strong>Program Administration, Financial Management &amp; Accountability<\/strong><\/h3>\n<ul>\n<li>Oversee implementation of the TWP Framework, including Violence Interruption, Compassionate Fund management, Care Coordination and Risk Mitigation Framework.<\/li>\n<\/ul>\n<ul>\n<li>Support grassroots partners in developing outreach strategies, intake processes, and case management support.<\/li>\n<\/ul>\n<ul>\n<li>Develop and update program materials.<\/li>\n<li>Lead and facilitate Coordination Meetings and other TWP-related meetings \u2013 ensure all TWP partners engage, share updates, challenges, and successes.<\/li>\n<\/ul>\n<ul>\n<li>Support, administer and manage the disbursement of all program funds.<\/li>\n<li>Oversee the tracking and reporting of all key program deliverables.<\/li>\n<li>Lead and support the hiring process for TWP staff.<\/li>\n<li>Ensure that all responsibilities are clearly outlined for all roles within TWP.<\/li>\n<li>Collaborate with City and other TWP partners to build relationships with institutional partners (TPS, TCHC, TDSB, TCDSB, RMYC, etc.).<\/li>\n<\/ul>\n<ul>\n<li>Explore and determine BRAVE and FOCUS referral pathways to the TWP model<\/li>\n<\/ul>\n<h3><strong>Monitoring, Evaluation, and Reporting<\/strong><\/h3>\n<ul>\n<li>Report and document all changes to the work plan and\/or budget.<\/li>\n<li>Report on all TWP expenses.<\/li>\n<li>Support grassroots partners to ensure quarterly reports, work plans and budgets are completed by the deadline.<\/li>\n<\/ul>\n<ul>\n<li>Support grassroots partners in achieving deliverables and reporting adequately.<\/li>\n<li>Ensure data collection and evaluation using City-approved tools (EGAP framework, case management systems).<\/li>\n<\/ul>\n<ul>\n<li>Support grassroots partners in meeting deliverables and submitting interim, quarterly, and final reports.<\/li>\n<\/ul>\n<ul>\n<li>Participate in quarterly sensemaking sessions with the City and partners to reflect on progress and outcomes.<\/li>\n<\/ul>\n<ul>\n<li>Compile and submit Activity &amp; Financial Reports on required timelines.<\/li>\n<li>Share key learnings about program implementation (successes, challenges, opportunities).<\/li>\n<\/ul>\n<h3><strong>Grassroots Engagement and Staffing<\/strong><\/h3>\n<ul>\n<li>Build and maintain rapport with grassroots staff.<\/li>\n<li>Develop and implement a wellness and retention plan for frontline staff (especially Violence Disruption Workers), grounded in trauma-informed and anti-oppression frameworks.<\/li>\n<\/ul>\n<ul>\n<li>Ensure that responsibilities are clearly outlined and defined for all roles within TWP.<\/li>\n<li>Support grassroots partners with supervision protocols, safety planning, and wellness supports.<\/li>\n<li>Strengthen grassroots staff capacity by identifying training opportunities and coordinating capacity-building initiatives.<\/li>\n<\/ul>\n<ul>\n<li>Collaborate with consultants to deliver Communities of Learning Practice activities.<\/li>\n<li>Provide mentorship and oversight for grassroots agencies (Rexdale and Jane &amp; Finch) through trustee agreements and\/or documentation processes.<\/li>\n<\/ul>\n<ul>\n<li>Support grassroots with conflict mediation should challenges arise.<\/li>\n<li>Support outreach and connections with community agencies; facilitate presentations to maintain partnerships for referrals.<\/li>\n<li>Support in informing case management system, outreach plan, and referral pathways for cohort participants.<\/li>\n<li>Ensure data collected by grassroots is secure and protected.<\/li>\n<li>Conduct ongoing check-ins with grassroots to highlight the support required for cohort members, leveraging social workers as part of the support system.<\/li>\n<\/ul>\n<ul>\n<li>Ensure culturally relevant and holistic approaches are embedded in practices.<\/li>\n<li>Monitor and revise eligibility, approval processes and guidelines for Compassionate Fund.<\/li>\n<li>Maintain tracking system for expenditure and participant use of Compassionate Fund.<\/li>\n<li>Support grassroots program leads in monitoring, recording, and distributing funds.<\/li>\n<li>Identify training opportunities for grassroots, provide updates on capacity-building supports.<\/li>\n<li>Leverage relationships and partnerships to implement training and capacity-building plan.<\/li>\n<li>Collaborate with training consultant to deliver Communities of Learning Practice activities.<\/li>\n<\/ul>\n<h3><strong>Communications &amp; Public Engagement<\/strong><\/h3>\n<ul>\n<li>Collaborate with the City and TWP partners on a public communications strategy aligned with TWP branding.<\/li>\n<li>Work with external organization(s) to develop a communications strategy, website, and social media.<\/li>\n<li>Assist in outreach and social engagement with community, youth, and families on social platforms.<\/li>\n<li>Ensure website information is up to date and reflects TWP branding and messaging.<\/li>\n<li>Represent TWP in citywide engagement initiatives, including the Peace March, National Day Against Gun Violence, MASTERPeace, and Elders Engagement.<\/li>\n<\/ul>\n<h3><strong>Risk &amp; Liability Management<\/strong><\/h3>\n<ul>\n<li>Lead ongoing development of the Risk Mitigation Framework to support VDWs and grassroots groups.<\/li>\n<\/ul>\n<ul>\n<li>Support and manage high-risk situations requiring violence interruption.<\/li>\n<li>Support extensive development of safety protocols for Lead Agency, grassroots, and VDWs.<\/li>\n<li>Monitor and facilitate annual evaluation (or as needed) of safety protocols, policies, and processes, identifying risks and revisions.<\/li>\n<\/ul>\n<p><strong>Qualifications and Assets<\/strong><\/p>\n<ul>\n<li>Degree\/diploma in Social Services, Child &amp; Youth Work, Social Work, or equivalent experience.<\/li>\n<li>Minimum 3 years\u2019 experience coordinating and delivering youth programs.<\/li>\n<li>Minimum 5 years\u2019 experience working with racialized youth.<\/li>\n<li>Minimum 3 years\u2019 staff supervision experience in a community setting.<\/li>\n<li>Strong knowledge of trauma, stigma, and culturally sensitive care.<\/li>\n<li>Proven experience in program development, implementation, evaluation, and reporting.<\/li>\n<li>Knowledge of community-based health services and needs of priority populations.<\/li>\n<li>Understanding of oppression and anti-Black racism, especially barriers faced by Black and racialized youth, particularly in the justice system.<\/li>\n<li>Experience working within anti-oppressive and anti-racist frameworks.<\/li>\n<li>Caseload management experience, including action planning, monitoring, and record keeping.<\/li>\n<li>Strong management skills (time, conflict, team facilitation, problem solving).<\/li>\n<li>Excellent communication and networking skills.<\/li>\n<li>Proficiency with MS Office, Internet, and email.<\/li>\n<li>Knowledge of Rexdale community considered an asset.<\/li>\n<li>Bilingual in French and English is a strong asset.<\/li>\n<li>Vulnerable sector security clearance required.<\/li>\n<li>Availability for flexible schedule, including some evenings and weekends.<\/li>\n<\/ul>\n<p><strong>Salary: <\/strong>$58,240 to $63,700 annually (based on experience),<\/p>\n<p><strong>Hours per week: <\/strong>35 (contract for 12 months with possibility of extension)<\/p>\n<p><strong>Starting date: <\/strong>ASAP<\/p>\n<p><strong>Application Due Date: <\/strong>September 23<sup>rd<\/sup>, 2025<\/p>\n<p>Please apply in confidence to:<\/p>\n<p>Stephanie McQueen, Human Resources Generalist Via email: <a href=\"mailto:stephanie.mcqueen@rexdalechc.com\">stephanie.mcqueen@rexdalechc.com<\/a><\/p>\n<p>Please quote \u201cTWP Project Lead\u201d in the subject line.<\/p>\n<p>We would like to thank all applicants for their interest;
