Working within our Interprofessional Primary Care Team (IPCT), you will provide clinical assessments, diagnosis, treatment, health promotion, disease prevention, and ongoing primary care services, with a focus on reducing barriers to care and improving health outcomes for underserved populations. This position requires fluency in Hindi, a commitment to culturally responsive care, collaboration with interdisciplinary team members, and a passion for serving diverse communities. The Nurse Practitioner will work across partner clinics within the North Etobicoke IPCT Collaborative, providing accessible, coordinated, and comprehensive primary care services in a variety of community-based settings. Responsibilities:

• Provide primary care to individuals and their families, on-site and in the community, taking into account risk factors (environmental, socio-economic, psychological, and medical) in a manner that is understandable to the client and, when applicable, their families.
• To provide health care, treatment plans, health education, and support for clients
• To participate in the development, implementation and evaluation of treatment, education, counseling and health promotion services for individuals, families and the community
• To order/interpret results of screening, diagnostic lab tests, X-rays, and ultrasounds
• Provide clinical care to pre- and post-natal pregnant women and their newborns, including breastfeeding support programs
• To determine the need for and implement health promotion, primary and secondary prevention strategies for individuals, families and communities
• Explore innovative service delivery approaches
• Advanced knowledge and proficiency in current, evidence-based methods and practices of primary care delivery with an emphasis on health promotion, risk reduction, and injury prevention
• To support and counsel clients in managing their chronic disease
• Familiarity with client-centered and self-management approaches to chronic disease prevention and management
• Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model
• Maintain and develop competence through appropriate continuing education methods

Qualifications and Assets:

• Bachelor of Nursing Degree with Extended Class Certificate of Competence from CNO in primary health care
• Registration with the College of Nurses of Ontario with professional liability insurance

Fluency in Hindi and English (both verbal and written) is required

• Affiliation with the Nurse Practitioners’ Association of Ontario (NPAO)
• Experience working as a Registered Nurse Practitioner in a community setting is an asset
• Knowledge of the Personal Health Information Privacy Act, Consent of Information/Records of Information, and Medical Records Management

Our team comprises diverse healthcare professionals committed to addressing the social determinants of health and achieving equitable health outcomes for all. As the Kinesiologist, you will play a role in delivering high-quality, accessible, patient-centered primary healthcare services to individuals and families in marginalized communities. Working within our Interprofessional Primary Care Team (IPCT). The position will involve delivering various group exercise programs in the community. The position will also involve developing pathways and programs to link individuals to ongoing access to exercise programs in the community. You will work independently and as part of an interdisciplinary team within a trauma-informed framework, with a focus on anti-black and anti-indigenous racism, discrimination, and oppression. This position requires a commitment to culturally competent care, collaboration with interdisciplinary team members, and a passion for serving underserved communities. This role will provide accessible care within Northwest Etobicoke from the Wheels to Wellness bus. Responsibilities:

• Provides comprehensive care to populations facing multiple barriers, ensuring access to those most vulnerable.
• Provide itinerant services from multiple locations to increase access for populations facing barriers to care.
• Work collaboratively within the team to develop and implement group-based exercise programs in the community (primarily older adults/seniors).
• Work collaboratively with the team to develop pathways/programs to link individuals to ongoing access to exercise programs in the community.
• In collaboration with the Manager and Director, develop and implement an evaluation for group programs and pathways.
• Participate in the ongoing evaluation of the group.
• Supports the Center’s student and volunteer placement programs and supervises students and volunteers as required.
• Contributes to the programs and services cycle of planning and evaluation, and participates in organization-wide planning, committees, and staff meetings as appropriate.
• Maintains and develops professional competence through appropriate continuing education methods.
• Contributes to activities to collect, analyze, and report on data and relevant information, and participates in research when appropriate.
• Safeguards client records, assures confidentiality of client information, and seeks to minimize risk.
• Identifies areas where the development of protocols and procedures is needed to improve client services or to promote more effective staff functioning.

Qualifications:

• Registered Kinesiologist from the College of Kinesiologists of Ontario or another recognized educational institution.
• Registered and in good standing with the Ontario Kinesiologist Association (OKA).
• Experience in program planning, development, monitoring, and evaluation, and working collaboratively with external partners.
• Extensive experience in leading group-based exercise programs, in particular for seniors.
• Experience providing comprehensive care to populations facing multiple barriers and with comorbidities, including mental health and addictions.
• Experience working in a team-based environment, in particular with other rehabilitation professionals.
• Experience working in a primary health care team or community setting is considered an asset.
• Thorough knowledge of and proficiency in current professional best practices.
• GLAD certification is an asset.
• Experience in pulmonary rehabilitation or cardiac rehabilitation is considered an asset.
• Experience with fall prevention programs is considered an asset.
• Demonstrated ability, commitment to, and knowledge of community health care.
• Experience working from a framework that is trauma-informed, with a focus on anti-black and indigenous racism, discrimination, and oppression, and ensuring a positive space.
• Ability to take initiative and work independently.
• Excellent interpersonal skills with clients in a culturally diverse practice population and with colleagues.

RCHC is recruiting for a part-time Nurse Practitioner to support the work of the Interprofessional Primary Care Team. The Nurse Practitioner will join our dynamic inter-professional team in providing holistic, comprehensive primary health care services to individuals, families, and communities in the North Etobicoke area. A Registered Nurse, Extended Class, in good standing with the Ontario College of Nurses, will use advanced assessment, physical examination, therapeutic and diagnostic skills to treat, counsel or refer clients. Using a collaborative, shared care approach, the Nurse Practitioner will work with other team members in health promotion and disease prevention, which may include individual, group, or community interventions. The Nurse Practitioner functions as a member of the primary care team in the provision of primary health care services to residents of the community, with a strong emphasis on illness and injury prevention and health promotion. The Nurse Practitioner will work within the interdisciplinary team at the Centre to address individual and community health issues, including providing holistic assessment of and care for patients and families with a focus on health promotion, disease prevention, health maintenance and rehabilitation A unique aspect of this role is the opportunity to provide care through the Centre's "Wheels to Wellness" mobile health program, delivering primary care services directly within the community and meeting people where they are. The Nurse Practitioner will play a vital role in improving access to care by providing outreach services in community settings and supporting individuals who may otherwise experience challenges accessing health services. In addition, the NP will support the Sickle Cell Disease program. Responsibilities:

• Provide comprehensive, client-centred primary health care to individuals and families at the Community Health Centre and through community outreach initiatives, including the Wheels to Wellness mobile clinic.
• Conduct comprehensive health assessments; diagnose and manage acute and chronic illnesses; develop and implement evidence-informed treatment plans; prescribe medications; and order, perform, and interpret laboratory tests, diagnostic imaging, and other investigations within the Nurse Practitioner scope of practice and College of Nurses of Ontario standards.
• Provide comprehensive primary care services across the lifespan, including health promotion, illness prevention, chronic disease management, prenatal and postnatal care, newborn assessments, breastfeeding support, and episodic care.
• Deliver specialized primary care for individuals living with sickle cell disease, including comprehensive assessment, ongoing monitoring, management of acute and chronic complications, pain management, preventive care, health education, and coordination of services across the continuum of care.
• Support individuals living with sickle cell disease during transitions between hospital and community settings, including post-discharge follow-up, coordination of specialist referrals, and facilitation of continuity of care.
• Collaborate with physicians, specialists, hospitals, Ontario Health Teams, community agencies, and other health care providers to ensure coordinated, integrated, and client-centred care.
• Develop trusting therapeutic relationships with clients and families by providing culturally safe, trauma-informed, anti-racist, and anti-oppressive care that recognizes the impact of systemic barriers and the social determinants of health.
• Provide health education, counselling, and self-management support to clients and families, with an emphasis on chronic disease prevention, healthy lifestyles, medication management, and disease prevention.
• Assess individual, family, and community health needs and participate in the planning, implementation, and evaluation of health promotion, disease prevention, education, counselling, and wellness initiatives.
• Identify opportunities for innovative and accessible models of care that improve health outcomes and reduce barriers to primary health care for underserved populations.
• Participate in community outreach activities, health promotion events, screening initiatives, and mobile clinic services to increase access to primary care for priority populations.
• Collaborate effectively as a member of the interprofessional primary care team, participating in case conferences, care planning, referrals, and shared decision-making to support holistic client care.
• Maintain accurate, timely, and complete clinical documentation in accordance with professional, organizational, and legislative requirements.
• Participate in quality improvement initiatives, program planning, evaluation activities, and the development of clinical policies, protocols, and best practices.
• Contribute to a positive learning environment by providing mentorship, supervision, and clinical education to students and other learners, as appropriate.

Qualifications and Assets:

• Bachelor of Science in Nursing (BScN) and current registration as a Registered Nurse in the Extended Class in good standing with the College of Nurses of Ontario (CNO).
• Current professional liability protection as required by the College of Nurses of Ontario.
• Current certification to prescribe controlled substances, or eligibility to obtain certification.
• Membership with the Nurse Practitioners' Association of Ontario (NPAO) is considered an asset.
• A minimum of three (3) years of clinical experience as a Nurse Practitioner in primary care, community health, or a related setting is preferred.
• Experience providing comprehensive primary care across the lifespan, including health promotion, disease prevention, chronic disease management, and episodic care.
• Experience providing care to individuals living with sickle cell disease, hematological conditions, or other complex chronic illnesses is considered a strong asset.
• Demonstrated knowledge of evidence-informed management of sickle cell disease, including chronic disease management, pain management, care coordination, and health promotion, or a willingness to develop specialized expertise in this area.
• Experience working with interdisciplinary teams using a collaborative, shared-care model.
• Demonstrated experience working with underserved, marginalized, and equity-deserving populations, including Black communities, newcomers, individuals experiencing poverty or homelessness, seniors, youth, and people living with complex health and social needs.
• Demonstrated commitment to anti-racist, anti-oppressive, trauma-informed, culturally safe, and client-centred care.
• Strong understanding of the social determinants of health and their impact on health outcomes, particularly for individuals living with sickle cell disease.
• Knowledge of the Community Health Centre Model of Care and a commitment to health equity and community-based primary health care.
• Demonstrated knowledge of health promotion, illness prevention, chronic disease self-management, and community development principles.
• Knowledge of the Personal Health Information Protection Act (PHIPA), informed consent, privacy legislation, and medical records management.
• Experience using electronic medical records (EMRs); familiarity with NOD, PSS, or comparable electronic medical record systems is considered an asset.
• Strong clinical assessment, diagnostic reasoning, and problem-solving skills with the ability to make sound clinical decisions independently.
