Registered Dietitian-Diabetes Educator

The Rexdale Community Health Centre (the “Centre”) is a multi-service centre that provides primary health care, settlement, community social support, and community development activities for women, children, families, youth, adults, and seniors of diverse cultures.

The Centre is currently recruiting a full-time Registered Dietitian-Diabetes Educator to work with a multidisciplinary diabetes education team, aiming to prevent diabetes and educate clients with diabetes on the management of the disease. The Registered Dietitian will also participate in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community.

  Key Responsibilities

  • Provide one-on-one nutrition counselling for clients with diabetes and chronic conditions.
  • Develop, implement, and evaluate individualized nutrition care plans tailored to diverse needs.
  • Facilitate weekly pre- and post-natal workshops and conduct monthly food demonstrations.
  • Lead and support community nutrition activities at both group and community levels.
  • Serve as a resource to colleagues, learners, and community partners on nutrition and diabetes care.
  • Ensure continuity of care through accurate record-keeping, case conferencing, and follow-ups.
  • Advocate for nutrition and diabetes-related community policies and initiatives.
  • Support student placements, new staff orientation, and in-service staff education.

Qualifications

  • Bachelor of Science in Food & Nutrition + accredited Dietetic Internship (community focus preferred).
  • Registration with the College of Dietitians of Ontario.
  • Experience in diabetes care; Certified Diabetes Educator (CDE) designation is an asset.
  • Minimum 2 years’ experience in a diverse, community-based health setting.
  • Strong understanding of the social determinants of health and cultural influences on diet.
  • Skilled at group facilitation, health promotion program delivery, and working with diverse populations.
  • Strong communication, interpersonal, and teamwork skills.
  • Flexibility to travel between sites (main office, satellite, and mobile diabetes clinics) and occasionally work evenings/weekends.
  • Knowledge of the Rexdale community and proficiency in a second language (e.g., Somali, Spanish, Urdu, Punjabi, Tamil) are assets.

Employment Details

  • Status: Full-Time, Permanent
  • Location: Rexdale (with travel between sites as required)
  • Salary Range: $65,327 – $78,704 per year (depending on experience)
  • Hours per week: 35 hours per week
  • Starting date: ASAP
