Job Summary:

The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds. RCHC is recruiting for a Full-Time Family Physician to support the work of the Interprofessional Primary Care Team (IPCT).

The IPCT is a collaborative initiative dedicated to providing culturally competent, comprehensive, and holistic primary healthcare services to marginalized communities, particularly focusing on the Black, Francophone, and racialized populations in Scarborough and Northwest Toronto. Our team comprises diverse healthcare professionals committed to addressing the social determinants of health and achieving equitable health outcomes for all.

As a Primary Care Nurse Practitioner, you will play a vital role in delivering high-quality, patient-centred primary healthcare services to individuals and families in marginalized communities. Working within our Interprofessional Primary Care Team (IPCT), you will provide clinical assessments, diagnosis, treatment, and preventive care to patients, with a focus on addressing the unique health needs and challenges faced by our target populations.

This position requires a commitment to culturally competent care, collaboration with interdisciplinary team members, and a passion for serving underserved communities. The Nurse Practitioner will be on our mobile “Wheels to Wellness” bus going through the community and meeting people where they are.

Responsibilities:

  • Provide primary care to individuals and their families, onsite and in the community, taking into account risk factors (environmental, socio-economic, psychological and medical) in a manner which can be understood by the client and, when applicable, their families.
  • To provide health care, treatment plans, health education and support for clients
  • To participate in the development, implementation and evaluation of treatment, education, counselling and health promotion services for individuals, families and the community
  • To order/interpret results of screening, diagnostic lab tests, x-rays and ultrasounds.
  • Provide clinical care to pre and post-natal pregnant women and their newborns, including breastfeeding support programs
  • To determine the need for and implement health promotion, primary and secondary prevention strategies for individuals, families and communities
  • Explore innovative service delivery approaches.
  • Advanced knowledge and proficiency in current, evidence-based methods and practices of primary care delivery with an emphasis on health promotion, risk reduction and injury prevention
  • To support and counsel clients in managing their chronic disease
  • Familiarity with client-centred and self-management approaches to chronic disease prevention and management
  • Thorough familiarity with therapeutic methods and practices based upon a health promotion/disease prevention model
  • Maintain and develop competence through appropriate continuing education methods

Qualifications and Assets:

  • Bachelor of Nursing Degree with an Extended Class Certificate of Competence from CNO in primary health care
  • Registration with the College of Nurses of Ontario with professional liability insurance
  • Affiliation with Nurse Practitioners’ Association of Ontario (NPAO)
  • Experience working as a Registered Nurse Practitioner in a community setting is an asset
  • Knowledge of Personal Health Information Privacy Act, Consent of Information / Records of Information and Medical Records Management
  • Familiarity with electronic medical records, e.g. NOD, PSS, is an asset
  • Proficient with computer software, including MS Office, Internet and email
  • Ability to work effectively both independently and as a member of an interdisciplinary Demonstrated ability to work well with women, youth, marginalized populations and diverse ethno-racial and cultural groups
  • Demonstrated experience in all aspects of program development and implementation
  • Awareness of and sensitivity to the health issues of a diverse community
  • Sound knowledge of health promotion and disease prevention is an asset
  • Excellent verbal and written communication skills
  • Demonstrated ability to multitask, be flexible, and be organized
  • Must be able to provide a criminal record check clearance
  • Ability and willingness to travel
  • Knowledge of the Rexdale community
  • Fluency in French, Spanish and Arabic is considered an asset.

Salary: $103,387 – $124,564 annually (depending on experience) plus HOOPP and benefits

Hours: 35 hours per week

Starting date: ASAP

Status: Non-unionized
