Rexdale CHC’s Case Management department and Client Support Workers provide intake assessments and support to clients who are experiencing difficulties accessing resources, gaining assistance with daily living needs, or wishing to improve their mental health. As part of our commitment to delivering community health support in Etobicoke, we work alongside individuals to better understand their circumstances and identify practical solutions that promote stability, well-being, and independence. Our approach recognizes that every person has unique experiences, goals, and strengths, allowing us to provide support that respects individual needs while encouraging meaningful progress.

We connect clients to services in the community, assist with advocacy, provide counseling to individuals experiencing life transitions, grief, and bereavement, and offer mental health support.
