You don’t need to know which program you need before reaching out. Simply tell us what has changed, who needs support, and what you’re hoping to find. Our team can help guide you to the right service.

If you’re looking for family medicine, mental health and counselling, diabetes education, or support for sickle cell disease, we can help you understand which option may fit your needs. We also offer community programs that encourage learning, social connection, and healthy living.

Before You Contact Us

You don’t need to prepare a detailed explanation. A few notes can help, such as:

• Whether you’re contacting us for yourself or someone else
• Any language, accessibility, or communication needs

It is important that you know your address because we will ask you for your postal code. Some of our services are based on postal code. You will also be asked about your insurance status: Do you have an Ontario Health Insurance Plan (OHIP)? Do you have coverage through the Interim Federal Health program? We can see you even if you don’t have a health card, but you will need to speak with our staff to determine if you are eligible for services.
