Registered Practical Nurse (RPN) – Foot Care

The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds. The Centre is currently recruiting an RPN – Foot Care to work with our Chronic Disease Management team.

Position Summary:

 The Foot Care Registered Practical Nurse (RPN) is responsible for providing comprehensive nursing foot care services to clients at risk of lower limb complications, including those living with diabetes, peripheral vascular disease, neuropathy, and other chronic conditions.

Working within a community health centre model, the Foot Care RPN promotes prevention, early intervention, and health equity through clinical assessment, treatment, education, and care coordination. The role supports lower limb preservation and reduces preventable complications, emergency visits, and amputations.

Key Responsibilities

Clinical Assessment & Direct Care

  • Conduct comprehensive foot assessments, including vascular and neurological screening.
  • Identify and stratify clients based on risk for ulceration and lower limb complications.
  • Provide nursing foot care services within RN scope of practice (e.g., nail and skin care, wound assessment, dressing changes).
  • Develop and implement individualized care plans.
  • Monitor and evaluate wound healing progress and adjust care as needed.
  • Initiate appropriate referrals to chiropody, primary care, vascular services, or specialty clinics.

Chronic Disease Management

  • Integrate foot health into chronic disease care plans, particularly for clients with diabetes.
  • Provide education on foot self-care, footwear, glycemic control, and risk reduction.
  • Reinforce self-management strategies during individual and group education sessions.

Interdisciplinary Collaboration

  • Work collaboratively with chiropodists, dietitians, nurse practitioners, physicians, social workers, and other allied health professionals.
  • Participate in case conferences and coordinated care planning.
  • Support continuity of care across internal and external partners.

Health Promotion & Outreach

  • Participate in community outreach and screening initiatives.
  • Deliver group education sessions related to foot health and prevention.
  • Support culturally responsive and accessible service delivery for priority populations.

Documentation & Quality Improvement

  • Maintain accurate and timely documentation in the electronic medical record.
  • Collect and report program data as required.
  • Participate in quality improvement initiatives related to lower limb preservation and chronic disease outcomes.
  • Adhere to infection prevention and control (IPAC) standards.

 Qualifications

  • Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse.
  • Certification in Advanced Nursing Foot Care
  • Minimum 2 years of recent clinical nursing experience (community health experience preferred).
  • Experience in wound care and chronic disease management
  • Current CPR certification
  • Strong communication, organizational, and documentation skills.
  • Proficient in the use of electronic medical records (EMRs) and standard computer applications (e.g., Microsoft Office).

Knowledge, Skills, and Abilities

  • Knowledge of diabetic foot complications, vascular disease, neuropathy, and wound management.
  • Ability to perform comprehensive lower limb assessments.
  • Strong clinical judgment and risk assessment skills.
  • Ability to work independently and within an interdisciplinary team.
  • Commitment to health equity, anti-racism, and culturally safe care.
  • Strong documentation and organizational skills.

Working Conditions

  • Clinic and community-based settings.
  • Prolonged periods of sitting and detailed clinical work.
  • May require occasional evening or weekend work.
  • Exposure to wound care environments requiring strict IPAC adherence.

 

 Salary: $28.10 – $31.30 per hour (based on experience)  with HOOP and Benefits

Starting date: ASAP
